Who is membership for?

Membership is designed for patients who have completed their Executive Comprehensive Exam and want ongoing physician partnership – not a one-off consultation.

How do the tiers differ?

Every tier shares the same clinical foundation. What changes is visit frequency (4, 6, or 12 a year), how directly you can reach Dr. Kouka, and how much – like BHRT and your Annual Executive Physical – is included rather than billed.

Can I change tiers later?

Yes. You can move up or down between Essential, Preferred, and Concierge as your needs change. Billing adjusts at your next quarterly cycle.

Is there a contract?

No long-term contract. Billed quarterly. Cancel anytime – your PHAP and labs remain yours.

Can my spouse join?

Yes. A spouse add-on is available at every tier for 50% of the member's monthly rate.

Does insurance cover this?

No. NOÜS is a cash-pay, physician-led practice. This allows Dr. Kouka to design plans around your needs – not insurance authorizations.
